Abstract

Four formulations of nicotine replacement therapy (NRT) have been approved by the PDA-the patch, gum, a nasal spray, and an inhaler. The goal when administering NRT is to prevent the onset of withdrawal symptoms and reduce nicotine cravings. The ideal dosage of NRT provides nicotine below the level absorbed while smoking, thereby discouraging the addiction that had been caused by smoking. NRT significantly improves smoking quit rates and therefore merits consideration in all patients attempting to quit, including most of those with coronary artery disease. The nicotine patch provides a steady serum level of nicotine. The other formulations can be given along with the patch for use when the craving is strong or withdrawal symptoms are troublesome.
