Evidence for an immunologic etiology for severe aplastic anemia and the role of immunosuppres-sion as therapy for aplastic anemia are discussed. In vitro marrow culture studies of granulocytic and erythroid colonies demonstrating immunologically mediated stem cell destruction are described. Case reports from the literature of autologous recovery after attempted bone marrow transplantation and the use of antilymphocyte globulins and cyclophosphamide for the treatment of aplastic anemia are reviewed. The response of four of nine severely aplastic children at the University of Minnesota to antithymocyte globulin and a second response to antithymocyte globulin seen in two patients after recurrence of severe aplasia are reported. Prospects for future studies of the immunologic aspects of aplastic anemia and for potential therapies are considered.
